In this section you will find some of our most frequently requested forms. You can print the form out, fill it in and send it to us. All forms are available in paper format and can be obtained by contacting Teachers' Pay and Pensions Team at the address or phone number at the bottom of the page. All forms are in PDF format and require Adobe Acrobat Reader which can be downloaded by clicking on the Adobe Acrobat link from our Downloads section.

CCU67 - self cert sickness form PDF 35.2 KB

This form should be used to certify illness for up to seven calendar days.

TR160 form notification of maternity absence PDF 93.6 KB

This form should be completed by ALL female teachers absent owing to maternity whether or not they are entitled to payment or are resigning.

TR160(A) form notification of adoption absence PDF 104 KB

This form should be completed by ALL teachers absent owing to adoption whether or not they are entitled to payment or are resigning.

SC3 form statutory paternity pay/paternity leave application PDF 41.5 KB

This form should be completed if you want to take time off work to support the mother of a baby or look after the baby.

SC4 form statutory paternity pay/paternity leave application for an adoptive parent. PDF 45 KB

This form should be completed if you want to take time off work to support your partner who is adopting a child.

This form should be completed to apply for additional statutory paternity pay.

TR142 form appointment of a part-time teacher PDF 24.9 KB

This form should be completed by schools appointing a part-time teacher. A TR23 form should also be completed.
